An application is valid
for 120 days after Florida KidCare receives it. If your children
do not get enrolled in MediKids, Healthy Kids, or the Children’s
Medical Services Network within the 120 days, Florida KidCare will
notify you and you will need to re-apply. An application that is
older than 120 days may still be used to determine if your children
are eligible for Medicaid.
Some Florida KidCare programs may have
limited space, and applications are accepted on a first-come, first-served
basis. When MediKids,
Healthy Kids and the Children’s Medical Services Network are
full, enrollment for these programs will close. Medicaid is always
open for children who qualify.
If enrollment for MediKids, Healthy Kids and the
Children’s
Medical Services Network is closed, Florida KidCare will check to
see if your children might be eligible for Medicaid. You will receive
more information if your children are eligible for Medicaid. If your
children are not eligible for Medicaid, you will need to re-apply
when MediKids, Healthy Kids, and the Children’s Medical Services
Network are open again.
How
to Print the Florida KidCare Application
The Florida KidCare application form and instructions
are available as a PDF file (Portable Document Format). You need
software called Adobe Acrobat Reader to print the form. You can get
Adobe Acrobat Reader for your computer here .
The application and instructions are a total of
4 pages, formatted for legal-size paper (8 1/2" x 14").
Please print the application on legal-size
paper (8 1/2" x 14"). Do not use letter-size
paper or reduce (shrink) the size of the application because
it makes the print so small that data-entry errors could occur
and cause delays in determining your child's eligibility.

Required
Documents
Changes to the Florida KidCare state law require
families to attach proof of income with your Florida KidCare
application. Your application is not complete without these documents. Please
send copies – do not send original documents.
If you don’t have these documents or if you
have questions, call toll-free 1-888-540-5437 to
find out what to do. Required income documents:
- Earned income.
- Most recent federal
income tax return, OR
- Pay stubs or wage statements from the last four weeks or
a letter from your employer that says how much money
you earned for the last four weeks, OR
- Most recent W-2 forms (Wage and Tax Statement).
- Unearned income.
If you get unearned income
(like social security benefits, disability benefits,
unemployment, workers’ compensation,
and veteran’s benefits), provide
a copy of the award letter or check stub.
- Child support.
If you get child support payments,
provide a copy of the court order,
or the most recent month’s
check received for each child, or a
written statement from the parent who
pays the child support.
Other documents:
- If your child is not a U.S. citizen, attach
a copy of the front and back sides of immigration documents
for each child you are applying for Florida KidCare. Examples
of documents for non-citizens are
listed in the application instructions and on this web site.
- If your child is an American
Indian or Alaskan Native, attach a copy of
the front and back sides of your child’s
tribal identification card or other similar tribal
documents.

Sending
Your Florida KidCare Application Package
There
are several ways to send in your application package: by mail,
by facsimile (FAX), by express/overnight mail, by hand delivery,
or by e-mail as scanned attachments.
